· Responsabilizar-se por todas as ocorrências de ordem técnica e legal no âmbito da farmácia;

· Assessorar equipes de enfermagem e médica, na utilização correta do acervo profilático e terapêutico;

· Controlar e conferir a entrada e saída dos medicamentos e materiais estocados no setor;

· Garantir o uso racional de medicamentos;

· Acompanhar o controle das prescrições de antibióticos e sua padronização;

· Analisar as prescrições médicas levando em consideração os seguintes parâmetros: interações medicamentosas, altas dosagens, cumprimento de protocolos de uso restrito;

· Acompanhar o fluxo das prescrições de produtos e realizar dispensação de medicamentos controlados;

· Validar o fracionamento e etiquetagem dos medicamentos sólidos orais;

· Garantir o suprimento de todos os medicamentos necessários à assistência dos pacientes;

· Realizar implantação dos processos introduzidos pela gestão de farmácia, implementar instruções de trabalho e monitorar sua efetividade;

· Realizar outras tarefas correlatas.

· Gerenciar a distribuição e dispensação de medicamentos de acordo com as prescrições médicas e as políticas internas do hospital.

·  Realizar a revisão de prescrições para garantir a segurança e a eficácia dos medicamentos prescritos.

·  Participar de equipes multidisciplinares para discutir planos de tratamento e otimizar a terapia medicamentosa. · 

Monitorar e avaliar os pacientes em relação ao uso de medicamentos, identificando e 

· resolvendo problemas relacionados a medicamentos.

· Gerenciar o estoque de medicamentos, garantindo a disponibilidade adequada e a segurança dos produtos.

· Prestar orientação e educação aos pacientes e profissionais de saúde sobre o uso correto dos medicamentos.

· Participar de comissões de farmácia hospitalar para desenvolver políticas e procedimentos relacionados à farmacoterapia.

· 

Realizar atividades de controle de qualidade e garantia da segurança dos medicamentos utilizados no hospital.

Requisitos e qualificações·        Ensino Superior Completo em Farmácia.  ·       Registro Ativo no Conselho de Classe.

·       Desejável experiência de 02 anos na área.

·       Conhecimento intermediário do Pacote Office.

·       Experiência prévia em farmácia hospitalar, preferencialmente em funções clínicas.

·       Conhecimento sólido em farmacoterapia, farmacologia e práticas clínicas.

·       Habilidades de comunicação eficaz e trabalho em equipe.

·       Capacidade de tomar decisões rápidas e precisas em situações de emergência

·       Conhecimento em sistemas de informação hospitalar e tecnologia farmacêutica.

·       Familiaridade com regulamentações de saúde e padrões de prática farmacêutica.
