Nossa cultura é acolhedora e colaborativa, com profissionais engajados em transformar vidas. Se você se identifica com esse propósito, venha fazer parte do nosso time!Responsabilidades e atribuiçõesRealizar o acompanhamento remoto de pacientes crônicos, agudos ou em situação de pós-alta hospitalar por meio de ligações, videochamadas ou plataformas digitais.Aplicar protocolos de triagem e classificação de risco, conforme orientações da equipe de enfermagem e médica.Monitorar sinais e sintomas relatados pelos pacientes, orientando conforme diretrizes estabelecidas.Registrar todas as informações dos atendimentos em prontuários eletrônicos ou sistemas próprios.Identificar sinais de agravamento clínico e acionar a equipe de saúde responsável ou encaminhar conforme necessidade.Fornecer orientações de saúde, educação em saúde e adesão ao tratamento, conforme protocolos padronizados.Participar de reuniões de equipe para discussão de casos, alinhamento de condutas e melhorias nos processos.Garantir sigilo e ética no tratamento das informações dos pacientes.Zelar pela qualidade e humanização do atendimento mesmo à distância.Atualizar-se continuamente sobre protocolos clínicos, ferramentas digitais e boas práticas de telemonitoramento.Requisitos e qualificaçõesCurso completo de Técnico de Enfermagem com registro ativo no COREN.Experiência prévia em atendimento ao paciente (presencial ou remoto).Conhecimento básico em protocolos de classificação de risco e sinais de alerta clínicos.Habilidade em comunicação clara, empática e humanizada, mesmo em atendimentos não presenciais.Facilidade com tecnologia (sistemas de prontuário eletrônico, chamadas por vídeo, aplicativos de saúde).Boa digitação e escrita para registros de atendimento em sistemas digitais.Informações adicionaisJornada de trabalho: de segunda a sexta-feira, das 18h às 22:30 (15m de intervalo) - Dias úteis.Modelo de atuação: presencial.Salário: R$ 1.642,00 Bonificação: R$ 300,00 (Cartão Flash)Convênio médico: Amil Convênio odontológico: Amil
