With a shared purpose “To Care,” Halton Healthcare is committed to delivering exemplary patient experiences, always, through compassionate, high‑quality, and integrated care. The organization is recognized for its strong focus on patient safety, innovation, and continuous improvement, and fosters a collaborative and inclusive workplace where people are supported to make a meaningful impact.

Job Description

The Spiritual Care Practitioner provides person-centred, holistic psychospiritual, emotional, existential, cultural, and religious care to patients, families, caregivers, and staff. As a member of the interprofessional team, the practitioner supports people through serious illness, suffering, loss, life transitions, spiritual or existential distress, dying, and bereavement across diverse religious, spiritual, cultural, and secular worldviews, without imposing personal beliefs. Practice follows Halton Healthcare policies, applicable legislation, and Canadian Association for Spiritual Care (CASC/ACSS) standards.

Key Responsibilities

• Clinical care: Completes psychospiritual assessments and provides therapeutic presence, supportive counselling, and individualized psychospiritual, emotional, existential, cultural, and religious interventions for patients, families, and staff; responds to acute distress and refers when needs exceed scope.

• End-of-life and bereavement: Supports patients and families before, during, and after death, including goals-of-care changes, after-death care, bereavement referrals, morgue viewings when requested, non-directive support related to Medical Assistance in Dying, and facilitation of requested spiritual, cultural, religious, or memorial practices.

• Inclusive care: Provides culturally humble and person-centred psychospiritual care across diverse religious, spiritual, cultural, Indigenous, humanist, and secular worldviews; connects patients with clergy, faith leaders, Elders, and community supports on request.

• Collaboration: Participates in rounds, family meetings, ethics discussions, and care planning; consults with physicians, nursing, social work, palliative care, ethics, and other interprofessional teams.

• Staff support: Supports staff experiencing grief, patient deaths, critical incidents, compassion fatigue, and moral distress; facilitates reflective practice and resilience-building conversations.

• Documentation and privacy: Documents psychospiritual assessments, interventions, care plans, and follow-up in the electronic health record; protects confidentiality and maintains professional boundaries.

• Education and quality: Educates staff, learners, and volunteers; contributes to policies, quality improvement initiatives, evidence-informed practice, and service activity reporting

Qualifications

• Master’s degree in Divinity, Theology, Spiritual Care, Pastoral or Psychospiritual Studies, or related discipline (or equivalent) preferred

• Completion two (2) advanced units of Clinical or Supervised Psychospiritual Education (CPE/SPE) or equivalent

• CASC/ACSS membership and Certified Spiritual Care Practitioner designation preferred; candidates progressing toward certification will be considered

• Registered with the College of Registered Psychotherapists of Ontario (CPRO), or in the process of registering

• Two years of hospital, hospice, palliative, or mental health spiritual care experience preferred; palliative and end-of-life experience strongly preferred

• Knowledge of grief, Medical Assistance in Dying, healthcare ethics, trauma-informed care, and diverse religious and non-religious worldviews

• Excellent listening, communication, and documentation skills; strong self-awareness and reflective practice

• Ability to work independently and within an interprofessional team

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
