ARBETSUPPGIFTER Som biståndshandläggare äldreomsorg på Socialförvaltningen, Hammarö kommun arbetar du utifrån en helhetssyn med brukaren i fokus. Arbetet innebär att utreda, bedöma och fatta beslut om samt följa upp insatser enligt Socialtjänstlagen.

I det dagliga arbetet innebär det enskilda möten, samverkan med interna och externa aktörer, anhöriga samt dokumentation. Vi utreder enligt modellen IBIC, Individens Behov i Centrum. Du arbetar självständigt men också i nära samarbete med kollegor och andra professioner och verksamheter. Du har nära stöd av ärendehandledare och kollegor i det dagliga arbetet

Beredskap i bostad vid storhelger ingår också i tjänsten. Årets tillfällen fördelas mellan samtliga biståndshandläggare.

KVALIFIKATIONER Du har socionomexamen eller social omsorgsexamen. Vi vill att du har goda kunskaper om gällande lagstiftning och det är meriterande om du har erfarenhet av myndighetsutövning eller av målgruppen.

Du har god kommunikativ förmåga och ett professionellt förhållningssätt som visas i ett gott bemötande.

Arbetet kräver att du har god datorvana, kan arbeta självständigt och strukturerat samt har lätt för att samarbeta. Du ska ha personligt engagemang samt god förmåga att uttrycka dig i tal och skrift. Du har B-körkort.

Tjänsten är en tillsvidareanställning på heltid. Tillträde: 2027-01-01, eller enligt överenskommelse.
