  • Complete comprehensive assessments of patients’ physical, functional, cognitive, psychosocial, and environmental needs.
  • Complete accurate OASIS assessments and admission documentation within agency-required timeframes.
  • Assess and document home health eligibility, including homebound status and the need for skilled services, when applicable.
  • Perform medication reconciliation, identify discrepancies or potential concerns, and communicate with the appropriate provider.
  • Develop individualized plans of care in collaboration with patients, caregivers, providers, and the interdisciplinary team.
  • Provide skilled nursing interventions and education related to diagnoses, medications, treatments, symptom management, and home safety.
  • Identify changes in condition or urgent concerns and initiate appropriate communication and follow-up.

  • Communicate admission findings, clinical priorities, and follow-up needs to the ongoing care team.
  • Collaborate with clinical leadership and quality assurance staff to resolve documentation questions and support continuity of care.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
