Vil du være med til at sikre god planlægning for borgere og kollegaer i et stærkt arbejdsfællesskab? Helhedsplejen Område Slots Bjergby søger en struktureret planlægger på fuldtid. Vi søger dig, der er uddannet social- og sundhedsassistent eller anden sundhedsfaglig baggrund, har flair for logistik og trives i en dynamisk hverdag. Du bliver en del af et engageret team, hvor vi samarbejder på tværs om at sikre en stabil drift og den rette hjælp til borgerne. Vi lægger vægt på det tværfaglige samarbejde, rehabilitering, trivsel og det gode arbejdsmiljø.

Dine primære arbejdsopgaver

  • Daglig og langsigtet planlægning af ruter og indsatser i omsorgssystemet Nexus samt KMD Opus-vagtplan.
  • Tæt koordinering og sparring med dine planlæggerkollegaer, områdeleder og personalet i helhedsplejen.
  • Sikre, at de rette kompetencer lander hos de rette borgere til tiden.

Vi søger en profil, der har

  • en uddannelse som SSA eller anden sundhedsfaglig baggrund
  • nogle års klinisk erfaring
  • erfaring med planlægning (en fordel, men ikke et krav)
  • stærke it-kompetencer - kendskab til Nexus og KMD Opus
  • gode kommunikationsevner og stærke tværfaglige samarbejdsevner
  • evnen til at bevare overblikket og roen i pressede situationer
  • en mentaliserende og positiv tilgang til både borgere og kollegaer
  • fleksibilitet, omstillingsparathed og evnen til at handle selvstændigt
  • gode evner inden for konfliktløsning
  • høj professionalisme, loyalitet og et stabilt fremmøde

Løn og ansættelsesvilkår Stillingen er en dagvagt fuldtidsstilling på 37 timer ugentligt. Ansættelsen sker efter gældende overenskomst med FOA - Social- og sundhedspersonale. Startlønnen forventes at tage udgangspunkt i indplacering som Planlægger på grundlønstrin 35 + årligt tillæg 5.000 kr. (31.03.00). Startlønnen udgør således 36.159,91 kr./md. ekskl. pension.
