Job Description

Employment Type: Full-Time Location: Salisbury South

There to care. Here for good. Aged care is so important. Some people forget that. You don’t and neither do we. Because helping people is what we were born to do. We’ve been a leading provider of aged care in communities across South Australia since 1953. That’s a long journey, and we’re proud of our stability and strength.

Now, we’re on a mission to continue to grow and improve our services and impact, hand in hand with our fantastic team. Here, every moment matters and every client counts. And so do you.

Join us as Clinical Care Partner and shape your future and ours.

A brighter future with Helping Hand, for older people and for you.

The Clinical Care Partner is a Registered Nurse who provides clinical expertise, advice, care management and oversight to support clients receiving care at home to access safe, high-quality and person-centred care.

As a key clinical advisor and escalation point for Care Partners and the broader home care team, you will support timely clinical decision-making and responses to risk, complexity and changes in a client’s condition. You’ll also provide direct nursing assessments and interventions and clinical care management for clients with complex needs.

You'll also make an impact by:

  • Providing timely, evidence-informed clinical advice to Care Partners and the home care team, including interpreting assessments, clinical information and hospital discharge documentation.
  • Supporting clinical decision-making for clients with complex needs, deterioration, palliative care or end-of-life care requirements.
  • Identifying, assessing and escalating clinical risks for clients with complex, high-risk or changing health needs.
  • Delivering nursing assessments, interventions and reviews within scope of practice, including wound care, medication-related support and clinical monitoring.
  • Providing clinical expertise to care planning, service reviews and reassessments while working collaboratively with Care Partners.
  • Contributing to clinical audits, incident reviews and quality improvement activities while promoting safe, evidence-informed practice.

What you'll bring To succeed in this role, you will have:

  • Current general registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (AHPRA).
  • A minimum of two years’ post-registration clinical experience, preferably in community, aged care or complex care settings.
  • Demonstrated clinical assessment, care planning, review and documentation skills, with the ability to identify clinical risk and exercise sound professional judgement.
  • Sound knowledge of professional nursing standards, scope of practice, Registered Nurse accountability and relevant legislative, regulatory and quality frameworks.
  • Demonstrated ability to coach, mentor and influence others through clinical leadership.
  • Current driver's licence and access to a reliable, roadworthy vehicle.
  • Current First Aid and CPR certification and NDIS Worker Screening Check, or willingness to obtain prior to commencement.

You'll stand out from the crowd if you have some of the following:

  • Experience in community aged care or in-home clinical service delivery.
  • Understanding of the Support at Home program and the Aged Care Quality Standards.
  • Experience in palliative care, chronic disease management or hospital-to-home transitions.
  • Experience providing competency-based training and supervision to community staff or contributing to clinical quality improvement.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
