Henry Ford Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide high-quality home health care services throughout designated Detroit communities. If you are passionate about patient-centered care and enjoy working independently while making a meaningful impact in patients' homes, we encourage you to apply.

48201, 48202, 48204, 48206, 48208, 48216, 48221, 48226, 48238

Schedule:

  • Full-Time
  • Monday-Friday
  • 8:00 AM - 4:30 PM

Responsibilities

  • Deliver skilled nursing care in patients' homes under the direction of the Registered Nurse and care team.
  • Monitor patient conditions and report changes to the appropriate healthcare providers.
  • Administer medications and treatments as ordered.
  • Educate patients and family members on disease management and care plans.
  • Complete accurate and timely documentation.
  • Travel throughout the assigned service area to provide patient care.

Qualifications

What You Will Need

Required Qualifications

  • Successful completion of a Licensed Practical Nursing (LPN) program accredited by the National League of Nursing.
  • Minimum of one (1) year of hands-on clinical LPN experience.
  • Current Michigan Licensed Practical Nurse (LPN) license in good standing.
  • NAPNES certification or successful completion of a pharmacology examination.
  • Reliable vehicle for daily travel to patient homes.
  • Valid Michigan driver's license and satisfactory driving record.

Preferred Qualifications

  • Previous Home Health Care experience.
