C. JOB SPECIFIC COMPETENCIES:

Demonstrates the required job specific technical/professional knowledge, skills and abilities. *

Assess to identify high risk patients (physical, psychological and social) sharing with the multidisciplinary team to collaboratively design a care plan with desired outcomes.

Plan healthcare pathways with interdisciplinary care team and intervene when necessary.

Coordinate and monitor the patient and triage for needed services across all service lines.

Provide education, i.e. disease process, treatments, self care skills etc. and support for patient and family.

Directs, monitors and supervises all clinical aspects.

Performs other duties as assigned.
