Physical Therapist FT – Home Health Full-Time Hillcrest Health & Living | Omaha Metro Area At Hillcrest Health & Living, our mission is simple: Inspiring People to Live Their Best Lives. We’re looking for an experienced Physical Therapist to join our Home Health team in a full-time capacity. This opportunity is ideal for a PT who values autonomy, relationship-building, and delivering meaningful care where patients are most comfortable — at home. If you’re someone who thrives on connection, clinical excellence, and creating a positive impact every day, we’d love to meet you. What You’ll Do

Provide skilled physical therapy services to home health patients across the Omaha metro area Develop individualized treatment plans focused on mobility, strength, safety, and independence Partner closely with nurses, providers, caregivers, and interdisciplinary teams to support patient goals Complete timely documentation and maintain compliance with all regulatory requirements Educate patients and families on exercises, safety, and continued care plans Build trusted relationships with patients while delivering compassionate, high-quality care

What We’re Looking For

Licensed Physical Therapist in the state of Nebraska (or eligible for licensure) Previous home health experience strongly preferred Minimum of 1+ years of PT experience required Strong clinical judgment, communication, and organizational skills Ability to work independently while staying connected with the care team Reliable transportation and valid driver’s license
