Founded in 1976, Home Nursing Foundation (HNF) is a leading charity that provides home healthcare services in Singapore, regardless of our patients' ability to pay. Dedicated to all-round support for our patients, HNF delivers comprehensive care programmes to look after patients' emotional well-being and healthcare needs.Our HNF Wellness Day Care & Rehabilitation Centres offer an array of social activities, social support and holistic healthcare services to help our elderly stay independent in their own community.

RESPONSIBILITIES

Client Care

  • Assist clients with Activities of Daily Living (ADL) such as like feeding, showering, toileting, moving around, elimination and transferring of clients
  • Monitor and record clients' vital signs in clinical system
  • Provide medication assistance
  • Prepare meals for clients based on diet plan from the Registered Nurse
  • Monitor and record clients' diet and ensure adherence to diet plan from Registered Nurse
  • Plan and Conduct Activities for all clients including clients with dementia
  • Support volunteer activities within the centre or at external premises
  • Support and conduct festival celebrations for clients
  • Escort clients between Centre and other destinations with or without transport.
  • Respond to both medical and non-medical emergencies and assist clinicians in delivering emergency care to clients
  • Integrate maintenance rehab activities into daily routine of seniors
  • Perform range of motion (ROM) exercises and mobility training

Centre Administration and Operations

  • Maintain the cleanliness of centre at all time
  • Ensure the kitchen and pantry areas are kept clean by adhering to the food handling guidelines set out by Singapore Food Agency (SFA).
  • Perform regular disinfection and cleaning of the premises, including equipment, furniture, and tools, as per the established schedule.
  • Assist with inventory management, including stocktaking and ordering cleaning supplies and other consumables.
  • Provide coverage for colleagues and support other Centres when required.

QUALIFICATIONS

  • Minimum O Levels
  • WSQ Certificate in Healthcare Support (Nursing Care) or relevant certification in Healthcare will be advantageous.
