The posting

JOB SUMMARY HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.

The Specialized Treatment for Optimized Programming (STOP) Area Six Program connects California Department of Corrections and Rehabilitation inmates and parolees to comprehensive, evidence-based programming and services during their transition into the community, with priority given to those participants who are within their first year of release and who have been assessed as a moderate to high risk to reoffend. Area Six includes San Diego, Orange, and Imperial counties. STOP subcontracts with detoxification, licensed residential treatment programs, outpatient programs, professional services, and reentry and recovery housing throughout the program area to assist participants with reentry and recovery resources.

The Community Service Representative (CSR) I is an in-person, field-based role that requires constant travel. The CSR I collaborates with a multidisciplinary team to deliver casework, assessment, referral, placement, and reentry services for individuals in the California Department of Corrections and Rehabilitation (CDCR), including those in the Community Transition Program (CTP) and other General Population (GP) inmates. The CSR I guides clients through service navigation to support health and recovery, from intake and placement into Community-Based Providers (CBP) through treatment and reentry. The role also includes maintaining documentation in compliance with CDCR and HealthRIGHT 360 policies. Reliable transportation is required due to the frequent travel demands of the position.

KEY RESPONSIBILITIES Case Management

Maintain an average caseload of 25+ CDCR clients. Provide learning experiences and clinical support to assist clients in meeting treatment goals. Develop Case Management Plans (CMP) and monitor client progress. Link clients to internal and external resources based on treatment needs. Maintain progress notes in clients’ files. Keep AOR informed of clients’ progress and develop exit and aftercare plans. Assist with referrals to community programming. Develop transition plans and coordinate with CDCR Releasing Institution staff. Travel to CBPs for outreach, case management, and assessment services. May require overnight stays. Correspond with AOR regarding readmissions and client transfers.

Placement

Assess, schedule, and coordinate placements for referrals. Coordinate transportation with the Transportation Coordinator. Match inmates to appropriate levels of care within a network of sub-contracted CBPs. Screen paperwork for treatment provision obstacles. Receive transitional plans from CDCR and County institutions. Communicate daily with Parole Agents. Conduct STOP outreach presentations in the community and prison yards. Coordinate referrals with other STOP Areas. Identify community capacity needs and revise placement requests.

Documentation

Maintain accurate and comprehensive client files. Enter data into electronic systems per guidelines from HealthRIGHT 360 and CDCR. Ensure accuracy, confidentiality, and timeliness of incident reports. Request timely reports from CBPs and report urgent incidents to a supervisor. Contact AOR for additional information as needed.

Administrative

Ensure ongoing communication with the CDCR personnel regarding clients. Attend the required training and meetings. Read and respond to emails when appropriate. Maintain compliance with registration or certification requirements, if applicable. Complete assigned training in a timely manner.

And perform other duties as assigned.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
