The posting

Is de ziekenhuisfarmacie jouw wereld, of wil je die van jou maken? Wij zoeken namelijk een apothekersassistent die met kennis en enthousiasme ons team komt versterken.

Wat ga je doen? Als apothekersassistent front-office werk je doordeweeks op de klinische afdeling of bij de polikliniek voor preoperatieve screening (POPA). Je hebt verschillende taken, je werkt met diverse collega’s en je bent het visitekaartje van de ziekenhuisapotheek.

Wanneer je werkzaam bent op de klinische afdeling ben je verantwoordelijk voor Apotheek Service Punt (ASP) opname, hierbij verifieer en registreer je de thuismedicatie in gesprek met de patiënt. Je voert ontslaggesprekken met de patiënten en je zorgt ervoor dat de patiënt met de juiste medicatie de deur uit gaat. Ook voer je medicatiebewaking uit, hier beoordeel en accordeer je alle voorgeschreven medicatieopdrachten en werk je nauw samen met artsen, verpleegkundigen en ziekenhuisapothekers. Je bespreekt de bijzonderheden en adviseert over de dosering. Wanneer je werkzaam bent op de POPA verifieer en registreer je de thuismedicatie in gesprek met de patiënt.

Vanwege de 24-uurszorg, zeven dagen per week, hebben wij vaste apothekersassistenten voor de avonddiensten, maar in geval van ziekte of verlof worden deze diensten ook gelijkmatig verdeeld onder de front-office apothekersassistenten. Pas als je geheel bent ingewerkt ga je ook meedraaien in de verdeling.
