Primaria de la comarca del Baix Empordà, provincia Girona, Cataluña: MÉDICO DE FAMILIA FUNCIONES: -Mantener la confidencialidad de la información a la que tenga acceso y el respeto a la intimidad y privacidad de los pacientes durante la asistencia. -Hacer diagnóstico, tratamiento y seguimiento de los pacientes que estén bajo su responsabilidad. -Tener conocimientos y habilidades para hacer el seguimiento de los pacientes frágiles, crónicos complejos (PCC), pluripatológicos o polimedicados. -Garantizar la continuidad del paciente durante todo el proceso asistencial en el lugar más adecuado: centro de salud, domicilio del paciente o atención continuada. -Colaborar con los profesionales de otros servicios asistenciales en el seguimiento de los pacientes. -Tener competencias en alguna técnica que se desarrolle en la atención primaria: ecografías, cirugía menor, crioterapia, etc. -Llevar a cabo actividades de promoción y prevención de la salud. -Trabajar con la evidencia científica disponible para mejorar la salud de los pacientes y de la comunidad. -Hacer y participar en las sesiones clínicas del centro periódicamente. -Participar en la consecución de los objetivos comunes estableciendo fórmulas de colaboración y compartiendo la información. -Participación en comisiones interdisciplinares con el hospital y centro sociosanitario si fuera el caso. -Formar parte de grupos de trabajo de Atención Primaria (cronicidad, ecografías, etc.) en su caso. -Participación en proyectos de investigación y formación continuada. -Realización de guardias en las áreas básicas del ámbito de Primaria. Requisitos - Licenciatura en Medicina i Cirugía con especialidad en Medicina Familiar y Comunitaria vía MIR u homologada o en trámite de homologación si la titulación se ha obtenido en un país extranjero.
