Über 11 000 Mitarbeitende aus 102 Nationen sorgen dafür, dass unsere Patient:innen gemäss den neuesten Entwicklungen, Methoden und Möglichkeiten der Medizin behandelt werden.

Für unsere Fachstelle Alters- und Pflegeheim und Kur suchen wir eine engagierte Teamleitung, die Freude daran hat, Mitarbeitende zu führen und zu fördern, in einem interprofessionellen Umfeld zu arbeiten und Prozesse aktiv weiterzuentwickeln. Die Fachpersonen aus dem Team beraten Patient:innen und Angehörige bei Fragen rund um den Übertritt in ein Alters- und Pflegeheim oder eine Kurklinik und unterstützen bei sozialversicherungsrechtlichen Fragestellungen sowie sozialen Problemlagen, die im Zusammenhang mit dem Spitalaustritt sichtbar werden.

Das erwartet Sie

  • Sie führen die sieben Mitarbeitenden des Teams und tragen die fachliche, personelle und organisatorische Verantwortung
  • Sie beraten und begleiten Mitarbeitende bei komplexen Fallkonstellationen und übernehmen bei Bedarf eigene Fallarbeit
  • Sie fördern die Zusammenarbeit mit den Behandlungsteams und externen Partner:innen, insbesondere Alters- und Pflegeheimen und Behörden, und sichern die fachliche Qualität der Arbeit
  • Sie analysieren und optimieren Abläufe im Austrittsmanagement und erarbeiten und aktualisieren Konzepte, Merkblätter und Arbeitshilfen

Das bringen Sie mit

  • Abgeschlossenes Studium in Sozialer Arbeit (BSc/BA/MSc)
  • Mehrjährige Berufserfahrung, idealerweise im Gesundheitswesen, in der Sozialhilfe oder im Erwachsenenschutz
  • Erste Führungserfahrung oder Motivation und Potential, in eine Teamleitungsfunktion hineinzuwachsen
  • Gute Kenntnisse des Schweizer Sozialversicherungswesens und der Nachsorgelandschaft sowie Interesse an Prozessmanagement, Veränderungsprozessen und der Weiterentwicklung von Abläufen
  • Gute Deutschkenntnisse, gute Französischkenntnisse sind von Vorteil
  • Selbstständige, strukturierte und lösungsorientierte Arbeitsweise sowie Belastbarkeit, Empathie und Teamfähigkeit
