Integra heeft een preventie – en zorgaanbod rond geestelijke gezondheidsthema’s inclusief middelenmisbruik, verslaving, gokken en gamen, epilepsie, psychosociale revalidatie, mentale beperking en forensische zorg en dit zowel voor kinderen, jongeren, volwassenen en ouderen.

Voor ons centrum Psychosociale revalidatie in Tongeren zijn we op zoek naar een collega voor een vervangingsopdracht. Onze werking richt zich op (jong)volwassenen met een psychische kwetsbaarheid en zorgnood waarbij moeilijkheden zich situeren op verschillende levensdomeinen. Via een intensief, doelgericht en tijdelijk hersteltraject op maat wordt ingezet op persoonlijk herstel en maatschappelijke integratie.

Taakomschrijving:

  • Je bent verantwoordelijk voor het in kaart brengen van de mogelijkheden en beperkingen van de cliënt op het vlak van functioneren in diverse levensdomeinen; wonen – werken – vrije tijd.
  • Je staat in voor het deskundig en doelgericht uitvoeren van ergotherapeutische handelingen en technieken met het oog op het behoud of verbetering van het functioneren.
  • Je zorgt voor de integratie van ergotherapeutische doelen in het persoonlijke herstelplan van de cliënt. Je doet dit in overleg met de cliënt en hebt oog voor de vertaling van deze evoluties naar de persoonlijke context van de cliënt.
  • Je neemt actief deel aan de teamvergaderingen en communiceert vlot en efficiënt met collega’s.
  • Je neemt verantwoordelijkheid op voor zowel individuele als groepsbegeleiding van onze cliënten.
