The posting

Love home health but ready for a change of pace from the road? Our Home Health LPN Clinical Coordinator role keeps you in clinical work, close to your patients and your field team, from our Oklahoma City office. You’ll be the person who keeps care moving: making sure visits happen, orders get where they need to go, and our nurses and therapists have what they need to take care of patients at home.

Interim HealthCare of Oklahoma City is a locally owned, Veteran and nurse owned agency that has served the OKC metro since 1999, with a 4.5 star CMS rating. Our LPN Clinical Coordinator works side by side with RN Case Managers, therapists, and office staff every day.

Schedule

  • Full time, Monday through Friday
  • 8:30amam-5:30pm
  • Office based at our Oklahoma City location

What you’ll do as our Home Health LPN Clinical Coordinator

  • Build and manage weekly visit schedules for nurses, therapists, and aides based on patient plans of care
  • Track visit frequencies and follow up on missed or rescheduled visits
  • Communicate with physician offices on orders, signatures, and plan of care updates
  • Keep RN Case Managers and the clinical team informed of patient changes, hospitalizations, and discharges
  • Take calls from patients, families, and field staff and route clinical concerns to the right person
  • Support new admissions by coordinating start of care visits with intake and field staff
  • Maintain accurate, timely documentation in our EMR
  • Help keep the team organized so field clinicians can focus on patient care

What we’re looking for

  • Active, unencumbered LPN license in Oklahoma
  • Home health experience strongly preferred
  • Strong organization and the ability to juggle several moving pieces at once
  • Clear, friendly communication by phone, email, and in person
  • Comfort working in an EMR and learning new systems
  • A team-first attitude and a heart for patients at home

Our Home Health LPN Clinical Coordinator works from our Oklahoma City office and supports patients and field staff across the metro, including Edmond, Norman, Moore, Yukon, Midwest City, Mustang, Guthrie, Choctaw, Wellston, Harrah, Piedmont, and Blanchard.
