Interim HealthCare of Oklahoma City is a locally owned, Veteran- and nurse-owned agency providing home health, hospice, and personal care across the Oklahoma City metro since 1999. Our Intake Coordinator is the first point of contact for referral sources, patients, and families, and the link that connects them to our clinical team.
Schedule
- Full-time, Monday through Friday
- 8:30 a.m. to 5:30 p.m.
- In-office at our Northwest Oklahoma City location (NW 56th & Portland)
- On-call one weekend per month | $100 per weekend

What You'll Do as Our Intake Coordinator
- Receive and process incoming referrals from physicians, hospitals, skilled nursing facilities, and community partners by phone, fax, and electronic portal
- Enter every referral the same day it's received, with the goal of getting patients started on care the following day
- Review each referral to make sure all required documentation is complete before care begins
- Confirm patient demographic information and run eligibility checks
- Enter referral and patient information accurately into our electronic medical record
- Coordinate with clinical managers and schedulers to set up start of care visits
- Communicate with patients and families with warmth and clarity, explaining next steps and answering questions
- Follow up with referral sources on status updates and any missing documentation
- Keep multiple referrals moving at once while maintaining accuracy and attention to detail
- Cover incoming referrals during your scheduled on-call weekend
- Track referral outcomes and support reporting for leadership
- Protect patient privacy in line with HIPAA requirements
What We're Looking For
- High school diploma or equivalent required
- 1+ years of customer service experience required; experience in a medical office or healthcare setting preferred
- Comfort with high call volume and a steady flow of incoming work
- Ability to prioritize and manage several tasks at the same time
- Availability for on-call coverage one weekend per month
- Strong written and verbal communication skills
- Accuracy with data entry and documentation
- Proficiency with computers, email, and electronic records systems
- Familiarity with medical terminology is a plus
