Registered Nurse (RN) Case Manager- Home Health

We are currently hiring a Registered Nurse (RN)/ BSN Case Manager- Home Health for our Lexington office. As an RN Case Manager, here’s a big-picture view of what you’ll do:

  • Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient/client care in accordance with home health laws and regulatory standards
  • Working closely with Director of Health Care Services and Administrator to do OASIS admissions, Recerts, Resumptions, DCs, case management and Coordinate with a team of LPNs, CNAs, aides and to ensure patient goals are met
  • Follow the Home Health orders from PCPs working closely with their office
  • Ensure medical supplies/equipment are delivered and staff are trained in care procedures
  • Assess patient/client needs, resolve issues, and ensure a high level of satisfaction
  • Effectively manage financial resources related to the delivery of quality patient/client care

A few must-haves for a Case Manager RN:

  • Graduate of an accredited nursing program and active RN license in Massachusetts or BSN
  • Minimum of 1 year of nursing experience in home healthcare/ OASIS admissions, recerts, resumptions and DC’s
  • At least 1 year of supervisory and caregiver management experience
  • CPR certification
  • Extensive knowledge of state and federal home health laws and regulatory standards
  • Excellent management, organizational, problem-solving, communication and interpersonal skills
