Registered Nurse (RN)- Home Care Visits

Schedule: This is a .8 position with flexible scheduling Monday-Friday between 8a-5p.

Location: Washington County, MN and St. Croix County, WI

Pay rate: $63.30-$74.20/ hour depending on experience plus mileage reimbursement

Position Responsibilities Include:

  • Perform assessments/follow up assessments and procedures, match employee skills with the clients' needs and develop working relationships with discharge planners and social service workers.
  • Obtain physicians plans of treatment and initiate execution, and provide services ordered by the physician as indicated in the care plan.
  • Counsel and educate the patient/family in meeting healthcare related needs, and effectively communicate with members of the interdisciplinary healthcare team.
  • Complete timely and accurate documentation/clinical notes including addressing the patient's progress.
  • Supervise paraprofessional and licensed professional team members (LPNs).Secure equipment and supplies and coordinate placement for use in patient's homes, and refer patients to other services as needed.
  • Participate in Quality and Performance Improvement (QAPI) program, along with the Office's Emergency Preparedness program as requested.
  • Follow accepted standards of nursing practice and use clinical decision making to efficiently and effectively manage an assigned patient population.
  • Access personal health information (PHI) necessary to fulfill the above duties and responsibilities.
  • All other duties as assigned.

Required Skills/Qualifications:

  • An active RN license in the states of MN and WI or ability to have both within 45 days of hire.
  • One year of experience as a Home Health Nurse
  • Preference given to candidates with a BSN degree
  • Current CPR/ BLS
  • Must have reliable transportation for patient visits including insurance
  • Experience with Oasis charting.
