On-Site Care Coordinator

Essential Functions:
- Responsible for appropriate and professional communication with clients and their families, community staff/managers and other healthcare providers as required. This includes addressing any time sensitive information as well as complying with all HIPPA guidelines including confidentiality.
- Meet with families, caregivers, and residents as needed to: provide information regarding on-site service to assist with obtaining new referrals and referring to Director of Healthcare Services for follow up.
- Identification of resources to address client’s health, daily living, and safety needs. Performing administrative tasks associated with service to clients including scheduling, logging, documenting all appointments and services; provide information, notices and regular communications with clients, business partner and Director of Healthcare Services.
- Monitoring client well-being, recommending resources and solutions as appropriate. Setting up all client appointments.
- Liaison and coordination with family members, designees, and providers.
- Creating and maintaining the client’s personal health care record for on-site location.
- Fulfilling all tasks in a thorough, timely, and reliable manner.
- May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
- Completes other assignments as requested and assigned.
Schedule:
- Monday through Friday
- Full-time
Minimum Education and Experience Requirements:
- 18 years of age or older.
- 2+ year experience in home care services.
- Previous leadership and sales experience preferred.
Knowledge, Skills and Abilities Required:
- Effectively reads and communicates in the English language.
- Basic computer skills.
- Minimum of 30 days of employment with Interim HealthCare.
