You'll work closely with adults, carers, Social Workers, health professionals, care providers, housing teams, and other partner agencies to help assess needs, coordinate support, promote independence, and ensure people receive appropriate services.

About the Role

As a Social Work Assistant (Adult), you'll provide practical and casework support to qualified Social Workers and wider Adult Social Care teams.

You'll contribute to assessments, reviews, support planning, safeguarding activity, and multi-agency working, helping ensure adults receive timely, appropriate, and person-centred support.

What you'll be doing:

  • Supporting qualified Social Workers with adult social care casework
  • Working directly with adults and carers to understand their needs, circumstances, and desired outcomes
  • Contributing to care and support assessments under the guidance of qualified professionals
  • Supporting reviews of existing care and support plans
  • Helping coordinate appropriate packages of care and community support
  • Undertaking home visits and community-based work where required
  • Gathering information from individuals, families, carers, and partner agencies
  • Supporting adults to access services that promote independence and wellbeing
  • Working with people with physical disabilities, learning disabilities, mental health needs, older people, or other care and support needs
  • Identifying safeguarding concerns and escalating them appropriately
  • Supporting safeguarding enquiries and actions under professional guidance
  • Working closely with health professionals, care providers, housing teams, and voluntary organisations
  • Participating in multidisciplinary meetings, reviews, and case discussions
  • Supporting carers and ensuring their needs are appropriately considered
  • Helping adults understand available care and support options
  • Supporting transitions between services where required
  • Maintaining accurate and timely case records
  • Preparing case notes, correspondence, and other documentation
  • Monitoring agreed actions and following up outstanding tasks
  • Supporting individuals with complex or changing needs
  • Promoting choice, independence, dignity, and control
  • Ensuring the individual's views and wishes are central to planning and decision-making
  • Managing a varied workload and completing tasks within agreed timescales
  • Maintaining confidentiality and professional boundaries at all times

Requirements

We're looking for a compassionate, organised, and professionally confident individual with experience supporting adults within health or social care.

You'll ideally have:

  • Previous experience within Adult Social Care, health and social care, community support, or a related service
  • Experience working directly with adults with care and support needs
  • Experience supporting qualified Social Workers or multidisciplinary teams
  • Understanding of the Care Act 2014 and person-centred social care practice
  • Knowledge of adult safeguarding principles and procedures
  • Experience contributing to assessments, reviews, or support plans
  • Experience working with adults with complex needs
  • Ability to identify and respond appropriately to safeguarding concerns
  • Strong communication and interpersonal skills
  • Ability to build positive professional relationships with adults, carers, and families
  • Experience working with health professionals, care providers, and partner agencies
  • Strong written communication and case-recording skills
  • Good organisational and time-management skills
  • Ability to manage competing priorities
  • Ability to work independently while following professional guidance
  • A person-centred and strengths-based approach
  • Good understanding of confidentiality and professional boundaries
  • Local authority Adult Social Care experience would be advantageous
  • Relevant health, social care, or social work qualification would be advantageous
  • Enhanced DBS clearance or willingness to undertake required safeguarding checks
  • Eligibility to work in the UK

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
