The Behavioral Health Coordinator provides non-clinical coordination support for behavioral health and primary care integration services at JPCHC's, Behavioral Health department. Based at the Arlington, this role serves as a key liaison between patients, providers, and Behavioral Health staff, helping ensure timely and accurate screening, scheduling, referral tracking, and communication across South Region clinic sites. This position does not require professional licensure but requires a strong working knowledge of behavioral health or social work practice and care coordination principles.
Essential Duties and Responsibilities
- Review patient charts in collaboration with clinical staff to determine appropriateness for Behavioral Health group services, applying established screening criteria.
- Coordinate scheduling of psychological and psychodiagnostic assessments, including communicating with providers, patients, and testing staff to confirm availability and requirements.
- Maintain and update scheduling templates, clinic hours, and referral pathways in the electronic health record so access points stay current as clinical capacity and programs change.
- Support the screening-to-referral workflow for substance use and related health needs — monitoring screening tools, alerts, and referral orders, and helping extend the workflow to additional providers and sites as capacity allows.
- Ensure a warm handoff occurs for every positive screen and track referrals through to the first visit, monitoring rates of completed screening, positive findings, and timely connection to care.
- Link patients to social services, housing, and recovery supports, and maintain up-to-date knowledge of available community resources.
- Participate in regular care team huddles with primary care providers and prescribers to review shared patients and coordinate follow-up.
- Serve as a primary point of contact for patients, providers, and clinic staff regarding Behavioral Health referrals, group enrollment, and assessment scheduling.
- Conduct patient-facing calls related to group intake and assessment scheduling, including gathering necessary information and answering routine questions.
- Track and manage the Behavioral Health referral queue to ensure timely follow-up and scheduling of services.
- Maintain accurate and timely documentation in Epic, including scheduling records, chart notes, and In Basket messages related to coordination activities.
- Maintain current knowledge of group program criteria, assessment referral pathways, and Behavioral Health scheduling protocols across South Region clinic sites.
- Collaborate with Behavioral Health providers and clinic staff at Arlington Avenue and other South Region sites as coverage needs require.
- Maintain confidentiality of all patient information in accordance with HIPAA and JPCHC policy.
- Perform other duties as assigned by the Practice Administrator or Director of Behavioral Health.
Qualifications
- Strong working knowledge of social determinants of health and the challenges facing underserved and low-income populations
- Familiarity with local community resources and social services systems
- Excellent interpersonal, written, and verbal communication skills
- Ability to pass a background check and drug screen in accordance with FQHC and regulatory requirements
- CPI (Non-Violent Crisis Intervention) certification preferred
- Bilingual or multilingual fluency preferred
Education and/or Experience
- Bachelor’s Degree in Social Work, Psychology, Human Services, or related field required or equivalent experience
- Minimum of two (2) years of experience working with individuals experiencing mental health, substance use, or social needs in a clinical, community, or social services setting required
- Must be credentialed according to the requirements of the evidence-based model(s) used
Required License
Valid driver’s license, reliable personal vehicle with current insurance, and a clean or passable driving record must be continually maintained in compliance with organizational standards. This position requires regular travel throughout the service area using a personal vehicle. Mileage reimbursement is provided per organizational policy.
Computer Skills
- To perform this job successfully, an individual should have moderate computer skills, including Microsoft Office (specifically Word and Excel) and other applications.
- Proficiency with electronic health records.
Physical Demands
- Must be able to sit, stand, walk, and drive for extended periods and may occasionally involve lifting materials up to 25 pounds.
