Zur Verstärkung unseres Teams suchen wir eine Dipl. Pflegefachperson HF / FH (40-100%) Pensum wählbar, keine geteilten Dienste Deine vielseitigen Aufgaben sind: Du bist in der direkten Pflege tätig und übernimmst bei komplexen Pflegesituationen, die Beratung und Anleitung der Kundinnen und Kunden, Angehörigen und Mitarbeitenden Du nimmst Bedarfsabklärungen mit dem Instrument InterRAI vor (Übernahme der Kosten für die Weiterbildung InterRAI durch die RegioSpitex Limmattal) Als Fallführung bist Du verantwortlich für die Planung und Durchführung des Pflegeprozesses bei Deinen Kundinnen und Kunden Du pflegst eine aktive Rolle in der Zusammenarbeit mit dem betreuenden, interdisziplinären Netzwerk und koordinierst die entsprechenden Dienstleistungen für Deine Kundinnen und Kunden Du bringst mit: Du verfügst über einen Abschluss als Dipl.
