Pflegefachperson HF mit Zusatzfunktion RAI Verantwortung Deine Aufgaben Fachgerechte und individuelle Pflege und Betreuung der Bewohnenden auf der konzeptionellen Grundlage des Modells der fördernden Prozesspflege Planung, Umsetzung und Evaluation pflegerischer Massnahmen unter Einhaltung der Kriterien des Pflegeprozesses und unter Einbezug der betroffenen Bewohnenden und deren Bezugspersonen. Mitgestaltung einer optimalen interdisziplinären Zusammenarbeit mit allen am Pflege\- und Betreuungsprozess Beteiligten Diensten Übernahme der Tagesverantwortung Sorgfältige und lückenlose Führung der Pflegedokumentation Exakte Erfassung der Pflege\- und Betreuungsleistungen sowie der verwendeten Pflege\- und Verbrauchsmaterialien. RAI Verantwortung: Durchführung und Sicherstellung der fristgerechten RAI\-Assessments gemäss gesetzlichen und betrieblichen Vorgaben. Verantwortung für die korrekte Einstufung des Pflegebedarfs sowie die Pflegeeinstufung der Bewohnenden. Erstellung, Überprüfung und laufende Aktualisierung der individuellen Pflegeplanung auf Basis der Assessment\-Ergebnisse. Sicherstellung einer vollständigen, fachgerechten und nachvollziehbaren Pflegedokumentation. Beratung und Unterstützung der Pflegefachpersonen bei der Anwendung des RAI\-Systems und der Pflegeplanung. Überwachung der Assessment\-Termine sowie Sicherstellung der Datenqualität und Vollständigkeit der Dokumentation.
