Die Spitex Fricktal AG gehört zu den führenden ambulanten Gesundheitsorganisationen der Region. Mit rund 100 Mitarbeitenden und 20 Lernenden sorgen wir täglich dafür, dass Menschen im Fricktal selbstbestimmt und sicher zu Hause leben können. Für über 600 Kundinnen und Kunden leisten wir jährlich rund 140'000 Einsätze – von der somatischen Pflege über psychiatrische Leistungen bis hin zur spezialisierten Palliative Care.

Zur Verstärkung unserer Teams in den Standorten , Möhlin und Rheinfelden suchen wir ab sofort oder nach Vereinbarung engagierte Persönlichkeiten, die Verantwortung übernehmen und gemeinsam mit uns die Spitex Fricktal von morgen gestalten.

Deine Aufgaben

  • Eigenverantwortliche, qualifizierte und ressourcenorientierte Pflege und Beratung unserer Kundinnen und Kunden
  • Bedarfsabklärung mit interRAI HC, Erstellen von Pflegediagnosen
  • Steuern und Überwachen des Pflegeprozesses als Fallverantwortliche
  • Enge Zusammenarbeit mit den interprofessionellen Teams
  • Zusammenarbeit mit Angehörigen, Behörden und involvierten Fachpersonen
  • Begleiten von Lernenden und Studierenden
  • Übernahme der Tagesverantwortung: Koordination und Ansprechperson des Tagesgeschäfts für Notfalleinsätze

Das bringst du mit

  • Abgeschlossene Ausbildung als diplomierte Pflegefachfrau/Pflegefachmann HF/FH oder gleichwertige Abschlüsse
  • Hohe Fach\- und Sozialkompetenz
  • Flexible und empathische Persönlichkeit
  • Gute Informatik Anwenderkenntnisse, Perigon\-Kenntnisse von Vorteil
  • InterRAI HC oder die Bereitschaft ein Kurs zu machen
  • PKW\-Führerschein
  • Wiedereinsteiger sind willkommen
