Ziel ist die Stabilisierung der häuslichen Situation und die angemessene Unterstützung der nahestehenden Bezugspersonen. Verantwortung für den Pflegeprozess und die Fallführung, inklusive Erstassessment und Reassessment, Erkennen von Veränderungen, Anpassung der Pflegeplanung und Evaluation der durchgeführten Massnahmen. Koordination von Dienstleistungen zur Förderung einer effizienten Zusammenarbeit aller Beteiligten in der Pflege und Betreuung. Förderung der Erhaltung und Wiedererlangung individueller Fähigkeiten unter Berücksichtigung ihrer Ressourcen und des sozialen Umfelds. Übernahme von Aufgaben in der praktischen Ausbildung von Studierenden und Lernenden, Förderung des Wissens- und Erfahrungstransfers im Team. Vertiefungsmöglichkeiten in folgenden Bereichen Berufsbildung  Palliativ Care Wundmanagement  Falls du bereits eine Weiterbildung in einem dieser Bereiche mitbringst, freuen wir uns über deine Qualifikation. Falls nicht, besteht bei uns die Möglichkeit, diese Weiterbildung zu absolvieren Das bringst du mit Abschluss als Pflegefachperson HF/FH  Fundiertes theoretisches und praktisches Wissen und Praxiserfahrung in der Pflege  Bereitschaft Verantwortung zu übernehmen und bei der Gestaltung der ambulanten Pflege mitzuarbeiten Freude an der Arbeit mit Menschen und vielfältigen pflegerischen Aufgaben Organisationstalent und empathische Kommunikation Führerausweis Kategorie B Ob mit langjähriger Berufserfahrung oder als Wiedereinstieg - bei uns bist du willkommen!
