Diese Festanstellung verbindet fachliche Spezialisierung mit hoher Selbstständigkeit und einem Arbeitsalltag, in dem Sie Klientinnen und Klienten sowie deren Angehörige nachhaltig unterstützen. Anforderungsprofil ##

  • Ausbildung als Fachfrau / Fachmann Gesundheit EFZ
  • Erfahrung in Spitex, Langzeitpflege oder Demenzpflege von Vorteil
  • Freude an selbstständigem Arbeiten im ambulanten Umfeld
  • Hohe Empathie, Geduld und ausgeprägte Sozialkompetenz
  • Strukturierte, zuverlässige und verantwortungsbewusste Arbeitsweise
  • Interesse an fachlicher Weiterentwicklung im Bereich Demenz
  • Führerausweis Kat. B für mobile Einsätze in der Region Basel

Aufgaben ##

  • Ganzheitliche Pflege und Betreuung von Menschen mit Demenz im häuslichen Umfeld
  • Einfühlsame Unterstützung von Angehörigen im Alltag
  • Mitwirkung bei Pflegeplanung, Verlaufserfassung und Pflegedokumentation
  • Beobachtung von Veränderungen und Weitergabe relevanter Informationen
  • Förderung von Selbstständigkeit, Orientierung und Lebensqualität
  • Zusammenarbeit mit Angehörigen, Ärzteschaft und weiteren Fachpersonen
  • Aktive Mitarbeit beim Ausbau des spezialisierten Demenz-Angebots
  • Tagdienst von Montag bis Freitag inklusive Feiertage
