Notre mission est d'apporter notre expertise en matière de soins et de soutien à domicile, pour accompagner les projets de vie des habitant-e-s du canton de Neuchâtel. Nous nous engageons fermement à rendre ces services accessibles à toute personne domiciliée dans le canton, indépendamment de son âge, de sa situation sociale ou économique. Nos prestations sont assurées par un réseau de micro-équipes soignantes, opérant de manière autonome et agile, et réparties au sein de 3 régions : Areuse, Doubs et Seyon.

Nous recherchons des

infirmiers (ères) 70-90%, en CDI

Vos missions

Rejoignez-nous pour accompagner nos clients dans leur parcours de soins et faire la différence au quotidien :

  • Évaluer et analyser les besoins des clients grâce à la démarche RAI-HC
  • Élaborer, en partenariat avec le client, un projet de soins personnalisé
  • Assurer les soins infirmiers thérapeutiques, de réadaptation et palliatifs, en les réalisant ou en les déléguant selon les besoins
  • Accompagner le client tout au long du processus de prise en soins, en tant que référent : soutien, information et éducation thérapeutique
  • Coordonner les activités de soins au sein de la micro-équipe pluridisciplinaire et avec les différents acteurs du réseau socio-sanitaire
  • Veiller au suivi administratif lié à la prise en soins
  • Participer activement à la formation continue et à la promotion des bonnes pratiques professionnelles

Votre profil ###

Vous êtes passionné(e) par les soins et le bien-être des clients :

  • Bachelor en soins infirmiers ou titre équivalent reconnu par la Croix-Rouge suisse
  • Autonome et proactif(ve), capable de collaborer efficacement au sein d-équipes pluridisciplinaires
  • Responsable et organisé(e), avec un sens aigu des priorités et de la rigueur dans le suivi des soins
  • Empathique et relationnel(le), capable de créer un lien de confiance avec les patients
