Vos tâches : \- Prise en charge de la responsabilité des cas : planification, pilotage et évaluation de l-ensemble du processus de soins, y compris les évaluations des besoins selon InterRAI-HC \- Soins et accompagnement professionnels et globaux de nos clients \- Responsabilité d-une tenue soigneuse et complète des documentations clients \- Collaboration active avec toutes les personnes impliquées dans le processus de soins \- Participation à l-équipe de compétence interne en soins palliatifs \- Soutien des chefs d-équipe dans les aspects professionnels et personnels \- Aide à l-organisation du quotidien professionnel et garantie d-un fonctionnement sans heurts
