Votre mission est d'assurer la répondance santé de plusieurs groupes de vie. Vous garantissez la continuité et la qualité des soins infirmiers, développez des actions de prévention et de promotion de la santé, et travaillez en étroite collaboration interdisciplinaire avec le personnel socio\\-éducatif et les partenaires du réseau, dans le respect du concept institutionnel. \\Mission :\\ Répondance santé et continuité des soins : \\- Être la personne de référence en matière de santé pour plusieurs groupes de vie \\- Assurer la continuité des soins infirmiers préventifs, curatifs, palliatifs et délégués \\- Identifier les situations critiques et y répondre de manière adaptée Prévention et promotion de la santé : \\- Mettre en place des actions de prévention, promotion de la santé et de dépistage \\- Promouvoir les mesures d'hygiène et les comportements favorables à la santé \\- Informer et sensibiliser les équipes et les personnes accueillies aux questions de santé Travail interdisciplinaire : \\- Collaborer étroitement avec le personnel socio\\-éducatif dans l'accompagnement \\- Participer aux rencontres interprofessionnelles et contribuer aux réflexions communes \\- Travailler avec les partenaires externes (médecins, hôpital, CMS, etc.) Suivi et qualité : \\- Tenir à jour les dossiers médicaux et les écrits infirmiers \\- Élaborer les synthèses santé et les projets de soins. \\Profil :\\ \* Diplôme d'infirmier·ère HES ou ES, ou titre jugé équivalent \* Intérêt marqué pour la prévention, la promotion de la santé et la santé communautaire \* Expérience en santé communautaire, un atout \* Aisance dans le travail interdisciplinaire avec des équipes socio\\-éducatives \* Sens des responsabilités, autonomie et capacité d'analyse \* Approche humaine, collaborative et orientée solutions. \\Nous offrons :\\ \* Des formations continues ainsi que des supervisions complémentaires \* Un environnement de travail structuré et collaboratif \* Des conditions de travail conformes à la CCT \* Une activité variée alliant soins, coordination et responsabilités professionnelles \* Un parking gratuit à disposition du personnel. \\Entrée en fonction :\\ à convenir \\Renseignements\\ : Mme P.
