Anforderungsprofil ##

  • Abgeschlossene Ausbildung als Fachfrau/Fachmann Gesundheit EFZ, Fachfrau/Fachmann Langzeitpflege oder dipl. Pflegefachfrau/Pflegefachmann HF
  • Mehrjährige Berufserfahrung in der Langzeitpflege
  • Fundierte Kenntnisse und praktische Erfahrung im RAI-LTCF
  • Selbstständige, strukturierte und verantwortungsbewusste Arbeitsweise
  • Belastbarkeit und Flexibilität, auch in anspruchsvollen Situationen
  • Freude an der Zusammenarbeit im Team und an der Übernahme von Verantwortung
  • Ausgeprägtes Qualitätsbewusstsein und eine sorgfältige Arbeitsweise

Aufgaben ##

Ihre Aufgaben in der Pflege:

  • Sicherstellung einer professionellen, individuellen und ressourcenorientierten Pflege und Betreuung
  • Früherkennung von Notfallsituationen sowie kompetentes und situationsgerechtes Handeln
  • Selbstständige, vollständige und lückenlose Führung der elektronischen Pflegedokumentation
  • Übernahme der Tagesverantwortung und Gewährleistung eines reibungslosen Ablaufs im Pflegealltag
  • Aktive Zusammenarbeit im interprofessionellen Team und Mitwirkung an der Weiterentwicklung der Pflegequalität

Ihre Verantwortung im Bereich RAI-LTCF:

  • Fristgerechte und fachgerechte Durchführung der RAI-LTCF-Assessments unter Einhaltung der gesetzlichen und betrieblichen Vorgaben
  • Verantwortung für die korrekte Einschätzung des Pflegebedarfs und die entsprechende Pflegeeinstufung
  • Erstellung, Überprüfung und Aktualisierung der individuellen Pflegeplanung auf Basis der Assessment-Ergebnisse
  • Fachliche Beratung und Unterstützung des Pflegeteams bei der Anwendung des RAI-Systems sowie bei der Pflegeplanung
  • Koordination und Überwachung der Assessment-Termine
