The posting

Le CMS READOM est un programme novateur et spécialisé dans la réadaptation à domicile, L’équipe pluridisciplinaire intervient durant trois semaines au maximum. Elle est composée d'ergothérapeutes, d'infirmiers, d'ASSC, de physiothérapeutes et d'auxiliaires de santé. Elle collabore avec le client, son entourage et le réseau de soin.

**Infirmier\-ère référent\-e READOM

80 \- 100%**

En tant qu’infirmier\-ère référent\-e, vous contribuez par vos compétences à la qualité optimale des interventions auprès des clients. Vous garantissez le lien entre le client, l’équipe pluridisciplinaire et les partenaires. Vous répondez aux besoins en soins, en assurant des soins de santé physiques et psycho\-gériatriques. Vous collaborez de manière interdisciplinaire dans le but d’une prise en charge globale du client.

Vous avez un Bachelor of Science HES\-SO en Soins infirmiers ou titre jugé équivalent, avec 3 ans d’expérience dans les soins aigus. Une expérience dans la réadaptation et une formation en évaluation RAI HC seraient des atouts. Vous êtes une personne responsable et autonome. Vous êtes à l'écoute, flexible et vous vous adaptez à n’importe quelle situation. Vous êtes à l’aise avec les outils informatiques. Vous êtes en possession d’un permis de conduire.
