- Выявляет граждан пожилого возраста и инвалидов, проживающих на территории обслуживания, нуждающихся в социальном обслуживании на дому. - Информирует пенсионеров и инвалидов об их правах, обязанностях и условиях оказания социальных услуг. - Принимает участие в оформлении документов на пенсионеров и инвалидов, нуждающихся в социальном обслуживании на дому. - Оказывает социально-бытовые социально-медицинские, социально-педагогические, социально-экономические, социально-правовые услуги гражданам, нуждающимся в социальном обслуживании на дому, предусмотренные перечнем гарантированных государством социальных услуг и не предусмотренные перечнем гарантированных государством социальных услуг, по выбору обслуживаемых - Вносит предложения по вопросу улучшения условий жизни и обслуживания пенсионеров и инвалидов. - Налаживает контакты с родственниками, соседями обслуживаемых. - Поддерживает контакт с фельдшером, лечащим врачом обслуживаемых. - Заполняет документацию. - дневник социального работника с отражением в нем предоставляемых услуг и дней посещения. - составляет отчет по результатам работы за месяц. - Принимает участие в семинарах, проф. учебах, совещаниях, связанных с деятельностью учреждения. - Участвует в социо-культурных мероприятиях, проводимых в учреждении. - Участвует в оказании услуг и (при необходимости) сопровождении инвалидов при оказании помощи на дому, в ином месте пребывания инвалида. - Оказывает инвалидам помощь, необходимую для получения в доступной для них форме информации о правилах предоставления услуги.
