Акушерско-гинекологическое отделение. Первичная оценка состояния после рождения. Сразу в родильном зале неонатолог проводит осмотр: оценивает дыхание, сердцебиение, цвет кожи, мышечный тонус, рефлексы и присваивает оценку по шкале Апгар. Это помогает быстро определить, требуется ли ребёнку неотложная помощь. 2. Мониторинг жизненно важных функций. Врач постоянно следит за температурой тела, частотой дыхания и сердечных сокращений, уровнем кислорода в крови, неврологическим статусом и другими ключевыми показателями. При ухудшении состояния осмотры проводятся чаще. 3. Оказание экстренной помощи. При угрожающих жизни состояниях (например, остановке дыхания или сердца) неонатолог проводит реанимационные мероприятия: искусственную вентиляцию лёгких (ИВЛ), интубацию трахеи, введение медикаментов. 4. Диагностика и лечение патологий. Неонатолог выявляет и корректирует различные нарушения: врождённые пороки развития, инфекции, дыхательные расстройства, метаболические сбои. Он назначает необходимые обследования (анализы, УЗИ, нейросонографию) и контролирует эффективность терапии. 5. Уход за недоношенными и маловесными детьми. Таким малышам требуется особый, щадящий режим. Их помещают в кювезы (инкубаторы), где поддерживаются оптимальная температура и влажность. Врач подбирает индивидуальные схемы питания и терапии. 6. Составление индивидуального плана наблюдения. На основе оценки состояния врач формирует программу медицинского сопровождения, особенно если есть факторы риска или выявленные отклонения. 7. Работа с родителями. Неонатолог консультирует маму по вопросам грудного вскармливания, ухода за новорождённым, профилактики заболеваний и вакцинации. Он объясняет, как правильно прикладывать к груди, как организовать режим и гигиену. 8. Проведение скрининговых обследований. Врач организует и контролирует неонатальные скрининги: на врождённые и наследственные заболевания (анализ крови из пятки), аудиологический скрининг (проверка слуха). 9. Контроль инфекционной безопасности. Неонатолог следит за соблюдением санитарных норм в отделении, чтобы предотвратить распространение госпитальных инфекций. 10. Взаимодействие с другими специалистами. При необходимости врач привлекает к консультации генетиков, неврологов, хирургов, офтальмологов и других узких специалистов для комплексного подхода к лечению. 11. Ведение медицинской документации. Все осмотры, назначения, проведённые процедуры и динамика состояния фиксируются в истории развития новорождённого или медицинской карте стационарного больного.
