Сбор информации и оценка состояния. Врач собирает жалобы и анамнез (в том числе от лиц, осуществляющих уход). Проводит физикальное обследование (осмотр, пальпация, аускультация) и интерпретирует результаты. Комплексная гериатрическая оценка. Это ключевой момент. Врач оценивает не только физическое состояние, но и функциональный статус (способность к самообслуживанию, передвижению, общению), психическое здоровье и социально-экономические условия жизни пациента. Выявление рисков и специфических синдромов. Гериатр ищет не только хронические заболевания, но и так называемые гериатрические синдромы (например, старческую астению), а также факторы риска конкретных осложнений: венозных тромбоэмболий, пролежней, падений. Диагностика и план действий. На основе оценки врач устанавливает диагноз (с учётом МКБ). Затем формирует индивидуальный план: профилактические, лечебные и реабилитационные мероприятия, а также план социально-психологической адаптации. В план могут входить меры по преодолению полипрагмазии (когда пациенту назначают слишком много лекарств). Назначение лечения. Врач подбирает лекарственные препараты (в том числе наркотические и психотропные) с учётом возраста, функционального статуса, диагноза и клинической картины, а также возможных нарушений функций органов. Также может назначаться немедикаментозная терапия (физиотерапия, ЛФК, психотерапия) и медицинские изделия. После назначения доктор контролирует эффективность и безопасность лечения. Организация дальнейшей помощи. При наличии показаний врач направляет пациента на консультации к узким специалистам или для специализированной (за исключением высокотехнологичной) помощи в стационар либо в дневной стационар. Реабилитация и адаптация среды. Гериатр разрабатывает план, который помогает адаптировать окружающую среду к возможностям пациента или наоборот функциональные возможности к среде (например, подбор средств передвижения, протезов, ортезов, коррекция сенсорных дефицитов). Он также контролирует выполнение реабилитационных мероприятий.
