Специалист по социальной работе, исходя из задач, возложенных на отделение: 1. Предоставляет социальные услуги в соответствии с уставной деятельностью Центра и исходя из потребностей клиента. 2. Организует первичный прием граждан. Осуществляет первичную проверку и анализ документов, свидетельствующих о проблемах граждан, обратившихся за получением социальных услуг. 3. Осуществляет сбор и обработку дополнительной информации, свидетельствующей о проблемах гражданина, обратившегося за предоставлением социальных услуг. 4. Проводит социальную диагностику, направленную на выявление ключевых проблем семьи, совместно с другими специалистами или самостоятельно. 5. Выявляет и оценивает индивидуальную потребность гражданина в различных видах социальной поддержки. 6. Консультирует граждан, о возможностях предоставления им социального обслуживания и мер социальной поддержки, об их правах и обязанностях в соответствии с законодательством Российской Федерации. 7. Ведет документацию и служебную переписку в соответствии с требованиями к отчетности, качеству ее предоставления, предоставляет заведующему отделением в установленные сроки отчеты о проделанной работе. 8. Организует взаимодействие с другими отделениями Центра, а так же осуществляет межведомственное взаимодействие с иными заинтересованными субъектами системы профилактики безнадзорности несовершеннолетних. 9. Разрабатывает совместно с клиентом план индивидуальной профилактической работы, проводит мероприятия, согласно плану, и осуществляет контроль за его выполнением. 10. Принимает участие в совещаниях отделения, социально значимых мероприятиях, благотворительных акциях, проводимых в Центре, в районе. 11. Осуществляет социальный патронаж (в том числе на дому) семей с детьми, оказавшихся в социально опасном положении. 12. Принимает участие в выявление семей с несовершеннолетними, находящихся в социально опасном положении совместно с субъектами профилактики безнадзорности несовершеннолетних, организует совместные рейды с представителями субъектов системы профилактики, а также принимает в них участие. 13. Проводит индивидуальную (групповую) профилактическую и разъяснительную работу с несовершеннолетними, склонными к девиантному или деликвентному поведению, а так же работу, направленную на социальную адаптацию несовершеннолетних.
