Для выполнения возложенных на него функций специалист по социальной работе обязан: - Осуществлять организацию первичного приема граждан; - Осуществлять первичную проверку и анализ документов, свидетельствующих о проблемах граждан, обратившихся за получением социальных услуг; - Выявлять недостающую информацию и (или) информацию, требующую дополнительной проверки; - Осуществлять сбор и обработку дополнительной информации, свидетельствующей о проблемах гражданина, обратившегося за предоставлением социальных услуг; - Выявлять и оценивать индивидуальную потребность гражданина в различных видах и формах социального обслуживания; - Консультировать граждан, обратившихся в учреждение, о возможностях предоставления им социального обслуживания, а также относительно документов, необходимых для получения определенного вида социальных услуг; - Осуществлять выбор технологий, видов и форм социального обслуживания, необходимых для достижения конкретной цели; - Согласовывать с гражданином цели оказания социальных услуг; - Осуществлять разработку и согласование с гражданином индивидуальной программы предоставления социальных услуг и мер социальной поддержки; - Планировать действия по достижению целей оказания социальных услуг; - Определять необходимый объем услуг по реализации индивидуальной программы - Организовывать оказание социально-бытовых, социально-медицинских, социально-психологических, социально-педагогических, социально-правовых, социально-экономических, услуг в целях повышения коммуникативного потенциала, услуг по социальному сопровождению граждан, срочных социальных услуг, а также мер социальной поддержки, осуществлять контроль за их исполнением путем проведения соответствующих выездов (выходов) к получателям услуг, составления соответствующих справок проверок, а также путем проведения проверки отчетов работников, документации, ведущейся работниками; - Надлежащим образом оформлять предоставление дополнительных платных услуг в соответствии с заключёнными договорами; - Обеспечивать своевременное внесение данных о получателях социальных услуг в АИС ЭСРН (в том числе индивидуальной программы, договора и иных необходимых документов, и сведений); - Участвовать в работе по оказанию государственной социальной помощи, согласно постановления Правительства Оренбургской области от 07.09.2020 № 753-пп О реализации Закона Оренбургской области от 16 апреля 2020 года №2180/581-VI-ОЗ О предоставлении отдельных видов государственной социальной помощи в Оренбургской области, а также в работе по предоставлению других государственных услуг учреждением, согласно утвержденным регламентам.
