Make a meaningful difference in patients’ lives by delivering accessible, evidence-based behavioral health care within an integrated primary care setting. In this role, you will support individuals experiencing mental health challenges, substance use concerns, and chronic medical conditions through personalized assessments, brief psychotherapy, and coordinated treatment planning. Working closely with medical providers, psychiatric consultants, and community partners, you will help patients navigate their care and achieve improved clinical outcomes. You will manage an assigned patient caseload, monitor treatment progress, and connect individuals with additional services when needed. This primarily remote opportunity combines virtual patient engagement with periodic in-person clinic visits in Oregon, offering the chance to contribute to a collaborative, patient-centered model of care.
Accountabilities:
- Screen and assess patients for mental health conditions and substance use disorders, identify individual needs, and facilitate engagement in appropriate treatment and follow-up care.
- Conduct patient assessments and develop individualized treatment plans in collaboration with the broader care team.
- Deliver brief, evidence-based behavioral health interventions, including behavioral activation, problem-solving treatment, motivational interviewing, and other clinically appropriate approaches supported by clinical technology tools.
- Support psychotropic medication management prescribed by medical providers by monitoring adherence, side effects, treatment effectiveness, and potential complications.
- Provide health behavior interventions and address social determinants of health to improve daily functioning, behavioral health outcomes, and overall medical well-being.
- Monitor patient symptoms and treatment response through in-person appointments, video consultations, and telephone encounters, adjusting support as clinically indicated.
- Educate patients about common mental health and substance use conditions, available treatment options, and strategies for managing their health.
- Coordinate referrals to in-clinic or community-based evidence-based psychosocial treatments and specialized mental health services when appropriate.
- Participate in regular, typically weekly, caseload consultations with psychiatric consultants and communicate resulting treatment recommendations to medical providers.
- Track patient follow-ups, treatment progress, and clinical outcomes using electronic health records (EHRs) and established clinical workflows.
- Maintain accurate and timely documentation of assessments, encounters, progress, and treatment recommendations for medical providers, psychiatric consultants, and other treating professionals.
- Identify patients who are not improving as expected and coordinate treatment plan adjustments with medical providers and psychiatric consultants, including referrals for more intensive or specialized care.
- Use established systems and workflows to identify patients who have disengaged from care and support their re-engagement in treatment.
- Contribute to continuous improvement by identifying operational inefficiencies and recommending enhancements to clinical processes and collaborative workflows.
- Participate in team meetings and maintain relationships with clinic clinicians and administrators through periodic in-person visits, generally about once per month.
- Meet monthly performance expectations established by clinical management and leadership.
- Master's degree or higher in Social Work, Psychology, Counseling, Rehabilitation, or another relevant discipline.
- At least 3–5 years of experience in behavioral health clinical interventions, case management, community care coordination, or a related area.
- Experience supporting diverse patient populations, including older adults covered by Medicare and individuals managing chronic medical and behavioral health conditions.
- Active, independent clinical licensure in Oregon as one of the following: Licensed Clinical Social Worker (LCSW). Licensed Professional Counselor (LPC). Licensed Marriage and Family Therapist (LMFT). Licensed Psychologist. Eligibility to enroll in Oregon's Medicaid and Medicare programs; current enrollment is preferred. Experience delivering evidence-based behavioral health interventions and coordinating care across medical, psychiatric, and community-based services. Proficiency with electronic health records, scheduling applications, remote telephone systems, and video conferencing platforms. Reliable, high-speed home internet suitable for virtual clinical services. Strong clinical assessment, documentation, communication, and care coordination skills. Ability to work collaboratively with multidisciplinary teams while managing an assigned caseload and meeting established performance targets. Willingness to attend periodic in-person clinic visits in Oregon, despite the primarily remote nature of the role.
