ASSC - Assistant(e) en soins et santé communautaire (H/F/D) - Soins à domicile

Persag SA recherche, pour l-un de ses partenaires dans le domaine des soins à domicile, un(e) ASSC motivé(e) afin de renforcer une équipe dans le canton du Jura.

Votre mission

  • Assurer les soins et l-accompagnement des patients à domicile dans le respect de leurs habitudes et de leur autonomie ;
  • Réaliser les soins de base et les actes médico-techniques relevant de votre domaine de compétences, sur délégation ;
  • Accompagner les patients dans les activités de la vie quotidienne ;
  • Observer l-état général du patient et être attentif(ve) à toute évolution de sa situation ;
  • Transmettre les observations pertinentes à l-équipe soignante ;
  • Participer à la continuité et à la qualité des soins ;
  • Favoriser le maintien de l-autonomie et du bien-être des personnes accompagnées ;
  • Collaborer avec les infirmiers, médecins et autres intervenants du réseau de soins ;
  • Assurer une documentation rigoureuse des soins et des interventions ;
  • Respecter les règles d-hygiène, de sécurité, de confidentialité et les procédures en vigueur.

Votre profil

  • Titulaire d-un CFC d-Assistant(e) en soins et santé communautaire (ASSC)
  • À l-aise dans le contact avec les patients et leurs proches ;
  • Bienveillante, respectueuse et attentive aux besoins de chaque personne ;
  • Autonome et capable de prendre des initiatives dans le cadre de ses compétences ;
  • Organisée et capable de gérer ses priorités ;
  • Dotée d-un bon sens de l-observation et de bonnes capacités de communication ;
  • Appréciant le travail en équipe et la collaboration interdisciplinaire ;
  • Flexible concernant les horaires et les différents types d-interventions ;
  • Titulaire du permis de conduire et disposant d-un véhicule, indispensable pour les déplacements entre les domiciles.
