Nous recherchons une Pflegefachperson HF pour une institution à Baselland.

Tâches ##

  • Assurer la prise en charge de personnes atteintes de démence.
  • Planifier et évaluer les processus de soins.
  • Collaborer avec les familles et les professionnels de santé.

Profil requis ##

  • Diplôme d'infirmier diplômé HF
  • Expérience professionnelle solide en soins de longue durée ou soins à domicile
  • Formation continue dans le domaine de la démence ou volonté de se qualifier dans ce domaine
  • Mode de travail autonome et responsable
  • Compétences sociales et professionnelles développées ainsi qu'une grande empathie
  • Permis de conduire catégorie B et volonté d'être mobile

Tâches ##

  • Soins et accompagnement adaptés aux ressources des personnes atteintes de démence dans leur environnement familial habituel
  • Planification, gestion et évaluation du processus de soins dans le cadre de la responsabilité du cas
  • Réalisation d'évaluations et d'entretiens de situation (RAI-HC)
  • Soutien des clients dans la vie quotidienne pour favoriser l'autonomie et la qualité de vie
  • Collaboration étroite avec les proches, les médecins et d'autres spécialistes
  • Service de jour du lundi au vendredi (y compris les jours fériés)
