Ambulant Ondersteuner Volwassenen

Als Ambulant Ondersteuner Volwassenen help je inwoners om zo zelfstandig mogelijk mee te doen in de maatschappij. Je ondersteunt volwassenen en gezinnen op verschillende leefgebieden, zoals werk, opvoeding, ontwikkeling, financiën, wonen en sociale contacten.

Samen met de inwoner en diens netwerk maak je een plan en kijk je welke ondersteuning nodig is. Je werkt systeemgericht, preventief en tijdelijk, met als doel de eigen regie en het normale leven te versterken. Daarbij werk je nauw samen met lokale voorzieningen en zet je waar nodig specialistische ondersteuning in.

Wat je gaat doen:

  • Ondersteunen van volwassenen en gezinnen op verschillende leefgebieden;
  • Samen met de inwoner en diens netwerk een ondersteuningsplan opstellen;
  • Ondersteuningsvragen integraal onderzoeken en kijken naar wat iemand zelf en met behulp van het eigen netwerk kan bereiken;
  • Ondersteuning plannen, organiseren en waar nodig zelf bieden;
  • Werken vanuit een systeemgerichte, preventieve en methodische aanpak;
  • Stimuleren van zelfregie, zelfstandigheid en deelname aan de maatschappij;
  • Samenwerken met lokale voorzieningen en andere betrokken professionals;
  • Waar nodig specialistische ondersteuning inzetten;
  • Evalueren van de ingezette ondersteuning en de behaalde resultaten;
  • Signaleren en bundelen van terugkerende hulpvragen om waar mogelijk collectieve ondersteuning te ontwikkelen of in te zetten;
  • Samen met collega’s verder vormgeven aan de ontwikkeling van de functie.

Wat jij meebrengt:

  • Een afgeronde relevante hbo-opleiding, bijvoorbeeld Social Work, Sociaal Maatschappelijke Dienstverlening of Pedagogiek (harde eis);
  • Minimaal hbo werk- en denkniveau (harde eis);
  • Relevante werkervaring in een vergelijkbare functie en kennis van relevante wet- en regelgeving;
  • Recente werkervaring in een vergelijkbare functie is een pré;
  • Een geldige SKJ-registratie is een pré;
  • Expertise in en affiniteit met de doelgroep ouderen is een pré;
  • Je werkt systematisch, methodisch en vanuit een integrale benadering;
  • Je kunt zowel zelfstandig als goed in teamverband werken;
  • Je bent flexibel, creatief, proactief en leergierig;
  • Je bent bij voorkeur per direct of op korte termijn beschikbaar;
  • Je bent beschikbaar voor 28 tot 32 uur per week.
