Gezinscoach

Als Gezinscoach ben jij het eerste aanspreekpunt voor jeugdigen en gezinnen die ondersteuning nodig hebben. Jij kijkt samen met het gezin naar wat er speelt en helpt om problemen vroegtijdig te signaleren en aan te pakken.

Je werkt in de leefwereld van het gezin: thuis, op school of in de wijk. Daarbij kijk je niet alleen naar het kind, maar naar het hele gezin en de omgeving eromheen. Je ondersteunt gezinnen kortdurend waar mogelijk en langduriger wanneer dat nodig is.

Wanneer er sprake is van complexe situaties of onveiligheid, durf je dit bespreekbaar te maken en werk je samen met het gezin en betrokken professionals aan passende oplossingen. Je stelt waar nodig een veiligheidsplan op en bewaakt de voortgang.

Je werkt nauw samen met scholen, collega’s binnen het sociaal domein en specialistische organisaties. Daarbij neem je, wanneer nodig, de casusregie en zorg je ervoor dat de juiste ondersteuning wordt ingezet.

Wat je gaat doen:

Je ondersteunt gezinnen en jeugdigen bij uiteenlopende hulpvragen. Per opdracht kunnen de werkzaamheden verschillen, maar je kunt denken aan:

  • Het begeleiden van gezinnen met opvoed- en gedragsproblematiek
  • Het voeren van gesprekken met jeugdigen, ouders en betrokken professionals
  • Het vroegtijdig signaleren van problematiek en inzetten van passende ondersteuning
  • Het opstellen en bewaken van veiligheidsplannen wanneer dat nodig is
  • Het uitvoeren van casusregie bij complexe situaties
  • Samenwerken met scholen, zorgaanbieders en andere partners
  • Het adviseren van professionals in het voorveld over preventie en ondersteuning

Wat jij meebrengt:

  • Een afgeronde HBO opleiding (bijvoorbeeld Social Work, SPH, MWD of vergelijkbaar)
  • Een geldige SKJ-registratie
  • Ervaring met gezinnen met gedragsproblematiek bij kinderen
  • Ervaring met het opstellen van veiligheidsplannen
  • Ervaring met oplossingsgericht werken binnen het sociaal domein
  • Sterke communicatieve vaardigheden en empathisch vermogen
