Klantmanager Regie en Mandaat

Als Klantmanager Regie en Mandaat ondersteun je volwassenen, minderjarigen en gezinnen bij complexe hulpvragen. Je kijkt daarbij niet alleen naar de individuele ondersteuningsvraag, maar naar het hele systeem rondom de inwoner. Je voert regie over trajecten waarin verschillende leefgebieden en vormen van ondersteuning samenkomen, zoals Jeugdhulp, Wmo, zorg, schuldhulpverlening en participatie.

Je bent de verbindende schakel tussen de inwoner, collega’s, hulpverleners en externe partners. Wanneer sprake is van een onveilige thuissituatie signaleer je risico’s, maak je deze bespreekbaar en zorg je dat de juiste partijen tijdig worden betrokken. Ook in complexe situaties houd je overzicht en zorg je dat ondersteuning integraal en doelgericht wordt ingezet.

Wat je gaat doen:

  • Regie voeren over complexe hulpverlenings- en ondersteuningstrajecten;
  • Ondersteunen van volwassenen, minderjarigen en gezinnen bij uiteenlopende en meervoudige hulpvragen;
  • Integraal kijken naar verschillende leefgebieden, waaronder Jeugdhulp, Wmo, zorg, schuldhulpverlening en participatie;
  • In kaart brengen van het systeem en netwerk rondom de inwoner;
  • Organiseren en coördineren van passende ondersteuning;
  • Verbinden van inwoners met betrokken hulpverleners, zorgaanbieders en andere externe partners;
  • Signaleren van risico’s en onveilige situaties binnen gezinnen en huishoudens;
  • Bespreekbaar maken van veiligheidsrisico’s en tijdig inschakelen van de juiste professionals;
  • Bewaken van de veiligheid en actief bijsturen wanneer de situatie daarom vraagt;
  • Bewaken van de voortgang en zorgen voor duidelijke afspraken tussen alle betrokken partijen;
  • Systemisch en methodisch werken vanuit een integrale benadering;
  • Beoordelen wanneer je zelf de regie voert, wanneer samenwerking nodig is en wanneer je juist kunt loslaten.

Wat jij meebrengt:

  • Minimaal hbo werk- en denkniveau, bijvoorbeeld vanuit Social Work, Sociaal Maatschappelijke Dienstverlening of Pedagogiek (harde eis);
  • Een geldige SKJ-registratie (harde eis);
  • Recente werkervaring in een vergelijkbare functie is een pré;
  • Kennis van relevante wet- en regelgeving binnen het sociaal domein;
  • Ervaring met complexe hulpvragen waarbij meerdere leefgebieden en professionals betrokken zijn;
  • Je werkt systemisch, methodisch en vanuit een integrale benadering;
  • Je kunt zowel zelfstandig als in teamverband goed functioneren;
  • Je beschikt over goede communicatieve vaardigheden;
  • Je bent flexibel, creatief, proactief en leergierig;
  • Je durft regie te nemen en kunt ook in complexe of onveilige situaties overzicht houden;
  • Je bent verbindend en weet verschillende professionals en belangen bij elkaar te brengen;
  • Je staat open voor andere manieren van werken die aansluiten bij het normale leven en de mogelijkheden van inwoners;
  • Beschikbaarheid per direct of op zo kort mogelijke termijn is een pré.
