Wmo Consulent / Casusregisseur

Als Wmo Consulent / Casusregisseur ondersteun je inwoners bij vragen rondom zelfredzaamheid, participatie en begeleiding. Je onderzoekt wat er speelt, kijkt naar de mogelijkheden van de inwoner en het eigen netwerk en beoordeelt wanneer ondersteuning vanuit de Wmo noodzakelijk is.

Je komt terecht in een gemeentelijke toegang die volop in ontwikkeling is. Naast het behandelen van je eigen caseload denk je actief mee over processen, werkwijzen en samenwerkingsafspraken. Dit vraagt om iemand die zelfstandig kan werken, snel productief is en zich gemakkelijk beweegt binnen een veranderende omgeving.

Wat je gaat doen:

  • Voeren van keukentafelgesprekken met inwoners over hun ondersteuningsvragen;
  • Onderzoeken en analyseren van hulpvragen rondom zelfredzaamheid en participatie;
  • Beoordelen van de noodzaak voor Wmo-begeleiding;
  • Indiceren en beschikken van maatwerkvoorzieningen binnen de Wmo;
  • Opstellen van zorgvuldige rapportages en beschikkingen;
  • Stimuleren van eigen kracht en het benutten van het netwerk en algemene voorzieningen;
  • Regie voeren op ingezette ondersteuning en monitoren van de behaalde resultaten;
  • Signaleren van veranderingen in de situatie van inwoners en tijdig bijsturen waar nodig;
  • Samenwerken met zorgaanbieders, netwerkpartners en andere betrokken professionals;
  • Zelfstandig beheren en behandelen van een caseload;
  • Bijdragen aan de ontwikkeling van processen, werkwijzen en samenwerkingsafspraken binnen de nieuwe gemeentelijke toegang.

Wat jij meebrengt:

  • Een afgeronde relevante hbo-opleiding, bijvoorbeeld Social Work, SPH, MWD, Ergotherapie of Verpleegkunde;
  • Aantoonbare werkervaring als Wmo-consulent;
  • Ervaring met het voeren van keukentafel- en/of gezinsgesprekken (harde eis);
  • Kennis van de Wmo, indicatieprocessen en relevante wet- en regelgeving;
  • Ervaring met indiceren, rapporteren, beschikken en regievoering;
  • Ervaring binnen een gemeentelijke toegang of het sociaal domein is een pré;
  • Je bent analytisch sterk en kunt zelfstandig professionele afwegingen en besluiten nemen;
  • Je beschikt over goede schriftelijke vaardigheden voor rapportages en beschikkingen;
  • Je bent samenwerkingsgericht en legt gemakkelijk verbinding met inwoners, hun netwerk en ketenpartners;
  • Je kunt snel zelfstandig een caseload oppakken;
  • Je bent flexibel en kunt goed omgaan met veranderende werkprocessen en nieuwe samenwerkingsafspraken;
  • Je bent per oktober beschikbaar (harde eis);
  • Je woont binnen maximaal 20 kilometer enkele reis van de werklocatie (harde eis).
