Wmo Specialist

Als Wmo Specialist ben jij verantwoordelijk voor het beoordelen van complexe Wmo-aanvragen en het bepalen welke ondersteuning inwoners nodig hebben. Jij kijkt verder dan de hulpvraag en onderzoekt wat iemand écht nodig heeft om zo zelfstandig mogelijk te kunnen functioneren.

Je werkt op het servicebureau waar aanvragen binnenkomen en beoordeelt of inwoners in aanmerking komen voor ondersteuning zoals beschermd wonen, ambulante begeleiding of andere voorzieningen. Je hebt contact met inwoners om hun persoonlijke situatie goed in kaart te brengen en kijkt naar mogelijkheden binnen de Wmo.

Je voert (her)onderzoeken uit en schakelt met verschillende professionals om tot een zorgvuldig en onderbouwd besluit te komen. Daarbij stel je duidelijke rapportages en beschikkingen op en zorg je voor een passende maatwerkoplossing binnen de wettelijke kaders.

Wat je gaat doen:

Je behandelt Wmo-aanvragen en werkt aan passende ondersteuning voor inwoners.

  • Het beoordelen van nieuwe en lopende Wmo-aanvragen
  • Het uitvoeren van (her)onderzoeken binnen de Wmo
  • Het voeren van gesprekken met inwoners om de situatie goed in kaart te brengen
  • Het indiceren van ondersteuning zoals beschermd wonen, begeleiding en dagbesteding
  • Het opstellen van rapportages en beschikkingen
  • Het opvragen van medisch advies waar nodig
  • Het op- of afschalen van zorg
  • Samenwerken met collega’s, behandelaren en andere partners

Wat jij meebrengt:

  • Een afgeronde HBO opleiding
  • Ervaring binnen de Wmo, bij voorkeur met beschermd wonen
  • Ervaring met diverse doelgroepen (zoals psychiatrie, verslaving, autisme en lichamelijke beperkingen)
  • Ervaring met Wmo 2015
  • Je staat stevig in je schoenen en kunt lastige gesprekken voeren
  • Analytisch vermogen en het kunnen stellen van kritische vragen
  • Goede communicatieve vaardigheden en een professionele houding
