Aufgabeninhalte:

* klinisch sozialarbeiterische Beratung und therapeutische Begleitung

stationärer Patientinnen und Patienten bei sozialen Problemstellungen

und psychosozialen Belastungen - bei Bedarf maßgebliche Einbeziehung

der nächsten sozialen Bezugspersonen

* Beratung und Begleitung in diversen Lebensbereichen (finanzielle,

berufliche, soziale Themen, Unterbringung, Tagesstruktur)

* Fallarbeit im Sinne des Case Managements

* Erstellung von Sozialanamnesen, Betreuungs- und Verlaufsberichte,

Ergebniserhebung inkl. Dokumentation

* interdisziplinäre Kooperation mit klinischen Abteilungen, Behörden,

Gesundheits- und Sozialeinrichtungen sowie Vernetzung mit den

relevanten lnstitutionen

* Teilnahme und Organisation interdisziplinärer Besprechungen und

Fortbildungsmaßnahmen

* Kooperation mit spitalsexternen Einrichtungen im Sinne eines

umfassenden Case Managements sowie Vernetzung mit den relevanten

Institutionen

* Leitung/Co-Leitung von Patientengruppen

Voraussetzungen:

* Diplom einer Akademie für Sozialarbeit oder abgeschlossenes Bachelor bzw.

Mag. (FH) Studium an einer FH für Soziale Arbeit (spätestens bei

Dienstantritt)

* fachliche Eigenverantwortlichkeit im Zuständigkeitsbereich, sowie

ausgeprägte Sozialkompetenz

* Bereitschaft für Außendienst (Führerschein B von Vorteil)

* Bereitschaft zur Integration in multiprofessionelle Behandlungsteams und

die Abteilung Klinische Sozialarbeit

* Bereitschaft zur Weiterbildung

* praktische Berufserfahrung und berufliche Vorerfahrung in der

psychosozialen Beratung sowie gute Kenntnisse der sozialen Landschaft

in OÖ erwünscht

* Englischkenntnisse von Vorteil

Einstufung: Wir bieten für diese Position ein Bruttomonatsgehalt von mindestens EUR 3.950,00

(Grundlage LD 14/02 + 75% GZ auf LD13) auf Vollzeitbasis, abhängig von den anrechenbaren

Vordienstzeiten, zuzüglich etwaiger Zulagen.
