As a Care Manager at Kidsfirstco, you will play a crucial role in coordinating and managing care services for children and their families. You will work closely with a team of dedicated professionals to ensure that each child receives personalized and comprehensive care. Your responsibilities will include developing care plans, monitoring progress, and collaborating with families and other care providers to achieve the best outcomes for the children. ## Responsibilities

  • Develop and implement individualized care plans for children.
  • Monitor and evaluate the progress of care plans.
  • Collaborate with families, healthcare providers, and educators to ensure comprehensive care.
  • Provide support and guidance to families regarding care options and resources.
  • Maintain accurate and up-to-date records of care activities and progress.
  • Participate in team meetings and contribute to the continuous improvement of care services.

Qualifications

  • 0-5 years of experience in a care management or related role.
  • Strong communication and interpersonal skills.
