Wil jij als zorgkundige méér doen dan zorg verlenen alleen? Op onze afdeling gerontorevalidatie begeleid je oudere patiënten tijdens een intensief en persoonlijk revalidatietraject. Je helpt hen om opnieuw zoveel mogelijk zelfstandigheid, vertrouwen en levenskwaliteit te verwerven.

Onze afdeling richt zich op de revalidatie van patiënten ouder dan 75 jaar met voornamelijk neurologische en orthopedische aandoeningen. Denk onder meer aan patiënten die herstellen na een beroerte, heupfractuur, gewrichtsoperatie, valincident of een periode van langdurige immobiliteit.

Je werkt in een multidisciplinair team waarin verpleegkundigen, artsen, kinesitherapeuten, ergotherapeuten, logopedisten, psychologen, diëtisten en sociaal werkers samen één doel nastreven: de patiënt zo zelfstandig en veilig mogelijk laten terugkeren naar de eigen woonomgeving of een passende vervolgsetting.

Wat houdt je functie in?

Als zorgkundige ben je een belangrijke schakel in het dagelijkse revalidatieproces. Je ondersteunt patiënten bij de activiteiten van het dagelijks leven, maar neemt deze niet automatisch over. Je stimuleert hen om binnen hun mogelijkheden opnieuw zelf handelingen uit te voeren.

Je observeert de patiënt nauwgezet, merkt veranderingen in functioneren of welzijn op en deelt deze informatie met het verpleegkundig en multidisciplinair team. Tijdens ieder zorgmoment draag je bij aan de afgesproken revalidatiedoelstellingen.

Je taken bestaan onder andere uit:

  • ondersteunen bij persoonlijke verzorging, toiletbezoek, aankleden, eten en drinken;
  • stimuleren van zelfzorg, mobiliteit en zelfstandigheid;
  • begeleiden van patiënten bij transfers, verplaatsingen en het gebruik van hulpmiddelen;
  • toepassen van veilige til-, transfer- en mobilisatietechnieken;
  • observeren en rapporteren van veranderingen in de lichamelijke, cognitieve, emotionele en sociale toestand van de patiënt;
  • ondersteunen bij maaltijden, met aandacht voor slikproblemen, voedingstoestand en specifieke afspraken;
  • bijdragen aan valpreventie, comfort, huidzorg en het voorkomen van complicaties;
  • begeleiden van patiënten met cognitieve problemen, neurologische uitval of gedragsveranderingen;
  • samenwerken met collega’s en andere disciplines rond de individuele revalidatiedoelen;
  • betrekken en ondersteunen van familie en mantelzorgers binnen de grenzen van je functie;
  • correct registreren en rapporteren in het patiëntendossier;
  • bijdragen aan een veilige, respectvolle en stimulerende afdelingscultuur.
  • Je beschikt over een visum en erkenning als zorgkundige. Daarnaast: heb je interesse in ouderenzorg en revalidatie; geloof je in de mogelijkheden van oudere patiënten; werk je geduldig, respectvol en stimulerend; observeer je nauwkeurig en rapporteer je helder; kan je omgaan met patiënten met complexe zorgvragen; ben je lichamelijk en mentaal in staat om patiënten veilig te ondersteunen; werk je graag samen in een multidisciplinair team; neem je verantwoordelijkheid op binnen je eigen bevoegdheden; sta je open voor feedback, opleiding en professionele ontwikkeling; kan je omgaan met wisselende zorgnoden en onverwachte situaties; draag je bij aan een open werkklimaat waarin collega’s elkaar ondersteunen en aanspreken. Ervaring binnen geriatrie, neurologische revalidatie, orthopedische revalidatie of een ziekenhuisomgeving is een pluspunt, maar geen absolute vereiste. Motivatie, leerbereidheid en een revalidatiegerichte basishouding vinden we minstens even belangrijk.
