Como parte do nosso compromisso com um modelo inovador de assistência, buscamos Fisioterapeutas Visitadores apaixonados pela transformação do cuidado à saúde no ambiente domiciliar, seguindo os princípios do modelo Buurtzorg, que prioriza o autocuidado e a manutenção das funcionalidades das pessoas em suas próprias comunidades.

Diagnóstico que Buscamos:

  • Formação em Fisioterapia e registro ativo no Conselho Regional de Fisioterapia (CREFITO).
  • Experiência prévia em atenção domiciliar ou saúde comunitária será um diferencial.
  • Paixão por promover o autocuidado e a autonomia dos pacientes, com uma abordagem centrada na pessoa.
  • Habilidade em comunicação e empatia, fundamentais para o estabelecimento de vínculos com pacientes e familiares.
  • Capacidade de trabalhar de forma colaborativa e multidisciplinar, integrando ações que promovam a saúde do paciente no seu ambiente familiar.
  • Proatividade na busca de soluções que promovam a qualidade de vida e a funcionalidade dos pacientes.

Proporcionar cuidados de alta qualidade, respeito, compaixão e empatia aos idosos, promovendo o seu bem-estar físico, emocional e psicológico, garantindo a segurança e a integridade do paciente, prevenindo e tratando eventuais problemas de saúde relacionados a mobilidade humana. O Fisioterapeuta com ênfase no tratamento de idosos, desempenha um papel crucial na educação do paciente, praticando cuidados adequadas e promovendo orientações e acompanhamento para o envelhecimento saudável e digno.

RESPONSABILIDADES

Atuar em consonância com os valores da organização, mantendo conduta focada em resultados do Cuidado, respeitando a individualidade de cada paciente, tornando a assistência personalizada.

Registrar no Prontuário do Paciente as informações inerentes e indispensáveis ao processo de cuidar, mantendo-os atualizados.

Ser representante da organização, zelando por suas premissas, prestando cuidado baseado em evidências científicas e melhores práticas.

Realizar o Cuidado Integral ao Paciente, cuidando com Empatia e Carinho, estabelecendo vínculos e considerar as suas necessidades biopsicossociais.

Garantir a continuidade da Assistência em Fisioterapia, por meio da execução do Plano de Cuidado estabelecido.

Distribuir adequadamente a agenda de consultas, cumprindo a carga horária e número de atendimentos, conforme o contrato estabelecido.
